PatientCare andWorkplace Challenges

Part1: AHA positionon workforce issues

Thepositionheldby AHA on variousworkforce issuesis consistentwith thefindingsof thecurrentresearch. However,AHA seemsto addressthemostobviouschallenges(suchas theshortageof healthcarestaff, overtime, mandatorylifts, andthestaffing patterns)whileforgettinga widerangeof challengesthat demoralizethehealthcareprofessionals.Forexample,AHA inadequatedeliveryof healthcareservicesto nurse shortage, butresearchshowsthatpoororganizational cultureis responsiblefor37 % of thelaborinefficiencyin thehealthcaresector(Fields, 2011).In addition, itwould be wrongto associatetheshortageof registerednursesentirelyto theincreasein thenumberof retiringbabyboomers. Thisis a demand-sideapproach,buta supply-side approachcan revealthatthenumberof studentsenrollingfornursingcoursehas reduced, which reducesthenumberof registerednursesavailableforrecruitment (American Association of Colleges of Nurses, 2015).Therefore,AHA positionis right, butthere is stilla needto identifytheunderlying causesof thechallengesreportedin thearticleusingmultipleapproaches.

Part2: Workforce challenge,unavoidable overtime

AHAstatesthatregisterednursesare oftenrequestedto providetheir servicesvoluntarilyafter theassignedschedulewith theobjectiveof addressingtheshortageof nurses.Theuseof thetermvoluntary is an understatementbecausenursesare actuallyforcedbytheadministratorsto prolongtheir scheduleswithout faircompensationfortheextendedduration.Forexample,Faller (2008) morethan 50 % of nursesare forcedto workformorethan 12 hoursa dayand17 % of them are forcedto doa mandatoryovertime.Thisimpliesthatthecorrecttermshould havebeen“mandatoryovertime”insteadof “unavoidableovertime”.Therefore, itis truethatnursesworkseveralhoursafter their assignedschedules, buttheydosoout of pressurefrom themanagement.Thisis could be themajorcauseof thedeclinein thequalityof careas wellas themedicationerrors.
